Your Roles & Responsibilities:
- Engaging in community outreach and education (seminars, speeches, workshops, etc. for the public)
- Serving as a spokesperson for the Elder Care Coordinator practice and developing relationships with key personnel in long-term care settings and other referral sources
- Visiting residential care facilities and other providers to build, maintain and facilitate relationships
- Work with our existing clientele to assist in all areas of care management in conjunction with our estate planning and long-term care planning activities
- The elder care coordinator works closely with other key planning services staff and attorneys for the facilitation of all aspects of long-term care planning, specializing in assisting clients with a need for long-term care and/or dementia-focused planning
- Assessing client’s independence and ability for self-care and coordinate services with client’s care providers
- Counseling client/family regarding aging issues
- Providing client and family education, support and guidance on matters relating to client’s health and long-term care
- Facilitating client and family access to community resources
- Assisting the client and family in understanding the long-term care and healthcare system, including limitations of public benefits
- Intervening and advocating for client’s legal right to quality health care and long-term care
Our Ideal Candidate:
You have 2 years’ experience in a long-term care setting or care management experience or related marketing experience in a long-term care setting or with an aging population.
You are comfortable working independently, proactive, possess strong people skills and are detail oriented.
You have excellent communication skills both verbal and written, and a compassion and commitment to working with our elderly clients and their families.
You love learning. Internal and external processes are constantly evolving and improving, and you are always looking for the opportunity to learn, grow, and contribute.
You are a fantastic multitasker. You will often be surrounded by competing priorities, and our ideal candidate can determine the difference between urgent and high priority, and act accordingly.
You are a self-starter. You’ll have many team members to collaborate with, but you’ll need to be able to take ownership of each of your projects from start to completion.
You are an excellent communicator. You have superior written and verbal communication skills.
Job Opportunity Details:
This will be a full-time position ($50,000-70,000 DOE) or could be a part-time position, based in our Syracuse office, with opportunity for professional development and career advancement.
